OHIP Assistive Funding Devices Program for Artificial Eyes

All residents of Ontario with a valid OHIP card are eligible for 75% funding of their custom ocular prosthesis or scleral cover shell through the Ministry of Health. An Assistive Devices Program application must be completed to partially cover prosthetic eye costs.

The Assistive Devices Program will also cover 75% of all associated maintenance of your new custom ocular prosthesis or scleral cover shell, lessening your out-of-pocket prosthetic eye costs.

If you are registered with the Ontario Disability Support Program or Ontario Works Program then we can obtain 100% coverage for you.

All Veterans Affairs (DVA) clients are eligible for 100% coverage. Prior approval is required.

All First Nations citizens with a valid status card are also eligible for 100% coverage. Prior approval is required.

All Workplace Safety & Insurance Board (WSIB) clients are eligible for 100% coverage as well as travel and overnight accommodations. This must be pre-authorized by your adjudicator prior to your scheduled appointment.

If you have private insurance with your work or company you will need to check your policy for exact coverage details. Although we cannot bill your Insurance Company of your behalf we will assist in every possible way to help you with the application forms and reimbursement process.
